Over the course of several years, we investigated and documented claims that antibiotic steroid eyeball injections were improperly compounded and supplied by the former Guardian Pharmacy for use during surgery by Jeffery Whitman, MD, and others at Key-Whitman in January & February 2017, and that the improper compounding created impure byproducts (formaldehyde and acetone) in the injections, resulting in a highly corrosive suspension that when injected
